About the role

The clinician provides speech and language services to clients with related complications, delivering care under the direction of the attending physician and in collaboration with families, nurses, or other responsible persons.

Responsibilities

• Develop speech therapy plans of care after consulting with the referring physician.
• Visit patients for assessment and evaluation; communicate with physicians before start of care and as needed; complete appropriate OASIS assessments.
• Provide education and instruction to team members, patients, and families.
• Document care and submit paperwork accurately and timely to maintain medical records.
• Participate in agency quality‑improvement programs as requested.
• Use company resources responsibly and educate the community about services to support growth and profitability.
• Accept direction from the Director of Clinical Services and meet or exceed established productivity standards.
• Coordinate with attending physicians or agency personnel regarding patient conditions; record information promptly in the EMR.
• Deliver speech/language pathology services per the interdisciplinary plan of care, recommending alternative communication methods, speech, and swallowing exercises.
• Attend regularly scheduled interdisciplinary team meetings to coordinate care, exchange information, problem‑solve, and receive staff support and education.
• Evaluate treatment outcomes and plan discharge as appropriate.
• Supervise Home Health Aides when needed.
• Respect patient rights in all care delivery.
• Report grievances, complaints, allegations of abuse, misappropriation of property, accidents, incidents, and emergencies to appropriate persons.
• Follow standard precautions and infection‑control procedures.
• Identify and respond appropriately to emergency situations and observe patient safety needs.
• Educate staff on psychosocial aspects as needed.
• Attend in‑services and meetings as required.
• Collaborate with contracted service providers as needed.
• Assist in developing and updating policies and procedures when requested.
• Maintain proper documentation for billing.
• Interface with patients, families, and staff to ensure customer satisfaction.
• Communicate effectively with patients, families, and other health‑care providers.
• Perform other duties as assigned.

Requirements

• Bachelor’s degree in Speech‑Language Pathology (required).
• Master’s degree in Speech‑Language Pathology (preferred).
• Minimum of one (1) year of experience in a home‑health setting (required).

Skills

• Mathematical Skills: Ability to add, subtract, multiply, and divide using whole numbers, fractions, and decimals; compute rates, ratios, and percentages.
• Language Skills: Ability to read, analyze, and interpret business periodicals, professional journals, technical procedures, or regulations; write reports, business correspondence, and procedure manuals; present information and respond to questions from leaders, team members, investors, and external parties.
• Other Skills and Abilities: Strong written and verbal communication; proficiency in English (read, write, speak); embrace hospice philosophy; ability to understand, read, write, and speak English.

Certifications, Licenses, and Registrations

Active, unencumbered Speech‑Language Pathologist license in the state(s) of employment is required.

Physical Demands and Work Environment

The role requires regular standing, walking, and manual dexterity, with the ability to lift and move objects up to 50 pounds. Visual acuity must include close and distance vision, color and peripheral vision, depth perception, and focus adjustment. The work setting involves exposure to bodily fluids, infectious diseases, and typical healthcare conditions. Routine use of standard medical equipment and clinical tools is essential.
