Home Health Speech Language Pathologist - PPV

Working under the direction of the attending physician, the clinician provides speech‑language services to patients in their homes, collaborates with families, nurses, and other team members, and upholds the Code of Ethical Conduct.

Responsibilities

• Develop speech‑therapy plans of care after consulting with the referring physician.
• Visit patients for assessment, evaluation, and OASIS documentation; communicate findings with the physician before and during care.
• Provide education and instruction to team members, patients, and families.
• Document care and submit paperwork accurately and promptly to maintain medical records.
• Participate in agency quality‑improvement programs and policy development.
• Use company resources responsibly and educate the community about services.
• Meet or exceed established productivity standards.
• Record patient information timely in the EMR and confer with physicians or agency personnel as needed.
• Deliver speech‑language pathology services per the interdisciplinary plan of care, recommending alternative communication methods, speech and swallowing exercises.
• Attend regularly scheduled interdisciplinary team meetings to coordinate care, exchange information, and problem‑solve.
• Evaluate treatment outcomes and plan appropriate discharge.
• Supervise Home Health Aides when required.
• Ensure care respects patient rights; report grievances, complaints, abuse allegations, accidents, and incidents.
• Follow standard precautions and infection‑control procedures.
• Identify and respond to emergency situations; observe patient safety needs.
• Educate staff on psychosocial aspects as needed.
• Attend in‑services, meetings, and collaborate with contract service providers.
• Maintain proper documentation for billing and ensure customer satisfaction through effective communication.
• Perform other duties as assigned.

Education and Experience

• Bachelor’s degree in Speech‑Language Pathology (required).
• Master’s degree in Speech‑Language Pathology (preferred).
• Minimum of one (1) year of experience in a home‑health setting (required).

Skills

• Mathematical: ability to add, subtract, multiply, divide, and compute rates, ratios, and percentages.
• Language: ability to read, analyze, and interpret professional journals, technical procedures, and regulations; write reports, correspondence, and manuals; present information and respond to questions.
• Strong written and verbal communication.
• Proficiency in English (read, write, speak).
• Understanding and embracing hospice philosophy.

Certifications, Licenses, and Registrations

• Active, unencumbered Speech‑Language Pathologist license in the state(s) of employment.

Physical Demands and Work Environment

• Regular standing, walking, and manual dexterity; ability to lift and move objects up to 50 lb.
• Visual acuity requirements: close and distance vision, color and peripheral vision, depth perception, and focus adjustment.
• Exposure to bodily fluids, infectious diseases, and typical healthcare‑setting conditions.
