Home Health Social Worker

About the role

The Medical Social Worker participates in interdisciplinary care for home health patients, assessing their social and emotional needs related to illness or injury. They develop personalized care plans, coordinate community resources, and evaluate patient and family responses to promote recovery and independence through comprehensive in-home care.

Responsibilities

• Assess social and emotional needs of home health patients related to illness or injury.
• Develop and implement personalized care plans.
• Coordinate community resources to support patient care.
• Evaluate patient and family responses to interventions.
• Provide guidance and oversight to a Social Worker Assistant.
• Maintain accurate and timely documentation.
• Support care coordination in accordance with company policies and healthcare regulations.
• Perform patient-facing duties, including adherence to TB screening requirements.
• Comply with driver safety policies for local travel.

Requirements

• At least one year of healthcare social work experience.
• Licensure as a social worker as required by state regulations.
• Valid driver’s license.
• CPR certification.
• Fluency in English.

Pay

Compensation ranges from $65,000 to $88,600 annually.
