About the Role
The Medical Social Worker provides holistic care to home health patients by assessing their social and emotional needs related to illness or injury. They develop personalized care plans, coordinate community resources, and support patients and families through ongoing evaluations and interventions. The role includes supervising a Social Worker Assistant, ensuring compliance with policies, standards, and regulations, and maintaining thorough documentation.
Responsibilities
- Assess patients’ social and emotional states, medical needs, and community resource requirements.
- Facilitate access to community services to address barriers to recovery.
- Communicate effectively with patients, families, and healthcare team members.
- Participate in interdisciplinary care planning and coordinate patient care activities.
- Monitor and evaluate the performance of social work staff, ensuring service quality and compliance.
- Maintain accurate documentation, including progress notes and case conferences.
Requirements
- Minimum of one year of experience in healthcare social work, home health, or hospice.
- Valid licensure as required by state law; current CPR certification.
- Knowledge of discharge planning, community resources, and medication administration.
- Strong organizational skills, ability to prioritize, and work under stress.
- Reliable transportation, valid driver’s license, and flexibility for local travel.
- Fluent in English, with ongoing licensure and continuing education.
