Responsibilities

• Administer medication in accordance with standards of practice.
• Encourage social/rehabilitative skills and encourage self-care and independence.
• Perform patient treatments in accordance with the plan of care.
• Auxiliary in teaching patients appropriate self-care techniques.
• Prepare clinical and progress notes to document care provided.
• Maintain awareness of patient conditions and report changes to the Clinical Director.
• Promote activities to achieve a safe and healthy environment.
• Practice aseptic technique when performing patient procedures/care.
• Maintain a productive work schedule according to agency standards.
• Attend and participate in patient care meetings, in-services, orientation, and staff meetings.
• Understand when to request supervisory assistance.
• Adhere to agency policies and procedures.
• Provide service in accordance with agency's philosophy, standards, policies, and scope of practice.
• Participate in performance improvement and adhere to HIPAA guidelines.
• Perform other duties as assigned.

Qualifications

• Licensed nurse in the State of Ohio, in good standing.
• Current BLS CPR Certification.
• One year or more experience as an RN/LPN in home health preferred.
• Must be able to read and understand various documents used by the agency including plan of care, flow charts, patient charts, agency policies and procedures.
• Knowledge of Medical Terminology is required.
• Requires math skills including ability to add, subtract, multiply and divide whole numbers.
• Must be able to think independently, recognize potential problems, gather information, and know when to contact a supervisor.
• Must be able to promote customer service and provide exceptional patient care, doing whatever we can to please our patients while staying within the rules of the agency and rules of each program served.
• Type and computer skills are required for this position.
