About the role

BayCare HomeCare is seeking a skilled Home Health Field RN to join our team in Sarasota, providing hands-on care in patients' homes. This full-time, exempt position offers a flexible schedule with occasional weekends and on-call responsibilities.

Responsibilities

• Provide hands-on care in the home for a group of patients consistently using the nursing process and clinical standards and protocols
• Oversee the activities of the multi-disciplinary team that provides care to the patient
• Monitor the provision of therapeutic services through written and verbal communication with all disciplines
• Ensure that all visits scheduled and provided are authorized and covered by physician orders
• Review patient visits weekly and provide plan to Patient Care Supervisor and team assistant, including anticipated weekend visits
• Inform the scheduler of any schedule changes and communicate caseload and patient care issues to Home Care Supervisor

Required certifications and licensures

• BLS (Basic Life Support)
• RN (Registered Nurse) License
• Driver's License (Florida driver's license required within 30 days of hire)
• Level II screening through the Florida Care Provider Background Screening Clearinghouse, managed by the Agency for Health Care Administration (AHCA)

Education

• Required: Associate's or Diploma in Nursing
• Preferred: Bachelor's in Nursing

Experience

• Required: 1 year Registered Nurse
• Preferred: 1 year Home Care
