Responsibilities

• Conduct comprehensive OASIS assessments and develop tailored plans of care in collaboration with primary care providers.
• Administer medications and treatments, anticipate patient needs, and make recommendations to optimize their well-being.
• Ensure timely and accurate documentation using our fully computerized software.
• Collaborate with in-office case managers to coordinate patient care.
• Empower patients and their families with essential health instructions, coaching, and counsel.

Requirements

• Valid and current Florida RN license.
• At least 1 year of Medicare Home Health RN experience.
• Current CEUs for HIV, Domestic Violence, and Alzheimer’s.
• Level 2 background screening (details at https://info.flclearinghouse.com/).
• Current CPR/BLS certification.
• Updated TB/PPD.

Schedule
