About the role

Safe Life Home Health Care, LLC is seeking a skilled Home Health RN Case Manager to join our 4-star team. Prioritize patient health and help prevent hospitalization while providing exceptional care in the comfort of patients' homes. Safe Life utilizes Andy AI, which assists our clinicians with OASIS completion and submission within 24 hours.

Responsibilities

• Provide skilled nursing care and case management in patients' homes
• Utilize Well Sky Kinnser EMR for documentation
• Complete and submit OASIS assessments within 24 hours with Andy AI assistance

Requirements

• Valid Illinois RN license
• Valid CPR Certification
• Valid Driver’s License and Auto Insurance
• Minimum 1 year of home health experience
• Proficiency in Well Sky Kinnser EMR
• Strong clinical assessment and communication skills
• Compassionate and patient-centered approach

Pay

$45 to $50 per hour OR $80,000.00 - $100,000.00 per year

Schedule

• Monday to Friday
• Weekends as needed
• Expected hours: 40 to 50 (less for part-time)
