Job Overview

Responsibilities

  • Makes patient and personnel assignments.
  • Captors patient care.
  • Coordinates communication with all physicians involved in a patient’s plan of care and other healthcare practitioners.
  • Integrates orders from all relevant physicians involved into the plan of care and ensures the orders are approved by the responsible physician.
  • Captors and integrates care provided by agency staff, including contractual providers.
  • Captors and coordinates referrals.
  • Assures that patient needs are continually assessed.
  • Assures reassessment of the patients’ needs is performed by the appropriate healthcare professional when there is significant health status change in the patient’s condition, at the physician’s request, and after hospital discharge.
  • Assures the development, implementation, and updates of the individualized plan of care in partnership with the patient, representative (if any), and caregiver(s).
  • Assures the provision of services as ordered by the physician as indicated in the plan of care, including patient and caregiver education and patient, caregiver, and family counseling.
  • Prepares clinical notes as appropriate.
  • Assumes responsibility for continuity, quality, and safety of services delivered in compliance with state and federal regulations.
  • Participates in activities relevant to professional services including development of qualifications and assignment of Agency personnel.
  • Participates in Agency-sponsored in-service training.
  • Recommends resources needed to provide care.
  • Collaborates on development and implementation of policies and procedures which guide and support the provision of care.
  • Collaborates with Agency leadership and appropriate staff to develop the organizational plan as it pertains to patient care.
  • If required to make patient visits, may work in patients’ homes in various conditions; possible exposure to blood, bodily fluids, and infectious diseases; ability to work a flexible schedule: ability to travel locally; some exposure to unpleasant weather: PRN emergency calls.

Requirements

  • Proven work experience as a Receptionist, Front Office Representative or similar role.
  • Graduated from an accredited college/university with a degree in the discipline of the professional license held.
  • Active Texas registered nurse (RN) license.
  • One year of home health care preferred.
  • Management experience preferred.
  • Current CPR certification.

Pay

$75,000.00 - $85,000.00 per year
