Responsibilities

  • Performing comprehensive nursing assessments in patients' homes.
  • Developing, implementing, and updating individualized plans of care.
  • Providing skilled nursing care, treatments, and patient education.
  • Collaborating with physicians, therapists, and interdisciplinary team members to coordinate care.
  • Monitoring patient progress and communicating changes in condition.
  • Educating patients and caregivers to promote confidence and independence.
  • Maintaining timely and accurate clinical documentation.
  • Delivering care in accordance with regulatory and organizational standards.

Qualifications

Required: Current, unrestricted Texas Registered Nurse (RN) license, minimum one (1) year of nursing experience, strong assessment and critical thinking skills, valid driver's license, reliable transportation, and current auto insurance, ability to travel throughout the assigned service area.

Physical Requirements: Sit, stand, bend, lift, and move intermittently throughout the workday. Must be able to lift, push, pull, and support 50–100 pounds, including assisting with patient transfers and moving equipment.

Preferred: Home Health or Hospice experience, OASIS documentation experience, Homecare Homebase (HCHB) experience, Associate or Bachelor's Degree in Nursing.

Pay

Compensation for this role will include a $7,500 Sign On Bonus. The base compensation will be determined based on a variety of factors, including qualifications, skills, competencies, and relevant experience.
