What You’ll Do

As a Home Health RN, you will serve as the clinical leader for your assigned patients, coordinating and delivering skilled nursing care in the home setting. Key Responsibilities Include:

• Conducting comprehensive nursing assessments and developing individualized Plans of Care.
• Managing a caseload of patients from admission through discharge.
• Performing start-of-care visits, routine visits, recertifications, and discharges.
• Evaluating physical, psychosocial, environmental, and functional patient needs.
• Collaborating with physicians, caregivers, and interdisciplinary team members to ensure continuity of care.
• Monitoring patient progress and adjusting care plans as needed.
• Identifying early changes in condition and intervening to prevent avoidable hospitalizations.
• Documenting all clinical findings, interventions, and outcomes accurately and timely.
• Ensuring safe, high-quality, patient-centered care in the home environment.

What We’re Looking For

• Current, unrestricted Massachusetts Registered Nurse (RN) license.
• Minimum of 1 year of clinical nursing experience.
• Home care is strongly preferred.
• Valid driver’s license, reliable transportation, and current auto insurance.
• Ability and willingness to travel within the Springfield service area (up to 50%).
• Strong communication, assessment, and critical thinking skills.
• Physical Requirements: Ability to sit, stand, bend, lift, and move intermittently. Must be able to lift 50–100 lbs.
