About the role
Skilled Nursing Registered Nurse (RN) providing comprehensive, patient-centered care in the home setting.
Responsibilities
- Deliver skilled nursing care in compliance with professional, regulatory, and organizational standards.
- Conduct thorough patient assessments and ongoing evaluations to determine needs and guide care plans.
- Develop, implement, evaluate, and revise individualized care plans collaboratively with physicians, patients, families, and the healthcare team.
- Follow physician orders, coordinate with therapy services, community agencies, and other providers.
- Apply clinical judgment and critical thinking to make independent nursing decisions.
- Educate patients, families, and caregivers on disease management, medications, treatments, and health promotion.
- Maintain accurate clinical documentation and communicate patient status effectively.
- Participate in on-call rotations, Quality Improvement activities, staff meetings, and training.
Qualifications
- BSN preferred or to be obtained within five years.
- Current Ohio RN licensure.
- BLS certification from AHA required.
- Minimum of one year of in-hospital or home care nursing experience.
Skills
- Proficiency with Microsoft Office; experience with EMR preferred.
- Strong organizational, communication, critical thinking, and time management skills.
- Interdisciplinary collaboration experience preferred.
Schedule
Full-time, onsite role with 40 hours per week.
