About the role
Skilled Nursing Registered Nurse (RN) providing patient-centered care in a home setting.
Responsibilities
- Perform comprehensive assessments and develop individualized care plans in collaboration with healthcare teams.
- Execute physician orders, coordinate with therapy services and community resources, and ensure safe, effective care delivery.
- Educate patients, families, and caregivers on disease management, medications, and health promotion.
- Maintain accurate clinical documentation and communicate patient status changes effectively.
- Participate in on-call rotation, quality improvement activities, staff meetings, and training.
- Adhere to professional, regulatory, and organizational standards in all care activities.
Qualifications & Skills
- Current Ohio RN license and BLS certification from AHA.
- Minimum of one year in-hospital or home care nursing experience; experience with interdisciplinary teams preferred.
- Proficiency in Microsoft Office; EMR experience preferred.
- Strong organizational, communication, critical thinking, and time management skills.
- Education: BSN preferred or to be obtained within five years of hire.
