About the role

The Registered Nurse (RN) provides comprehensive, patient-centered skilled nursing care in a home setting.

Responsibilities

• conducting thorough patient assessments
• developing and updating individualized care plans
• executing physician orders
• coordinating with therapists and community resources to ensure quality care
• using critical thinking and clinical judgment to make independent decisions
• educating patients and families on disease management and treatments
• maintaining detailed documentation
• communicating effectively with the healthcare team
• participating in quality improvement activities, staff meetings, and training
• may be involved in on-call coverage

Qualifications

• current RN license in Ohio
• BLS certification
• at least one year of relevant experience
• proficiency with Microsoft Office and EMR systems
• A BSN is preferred or must be obtained within five years

Schedule

This role is full-time, onsite.
