Home Health Rehabilitation Manager - Physical Therapist (PT), Full Time,

Key Responsibilities

• Perform in-home assessments and develop individualized treatment plans based on patient needs
• Provide one-on-one therapy sessions and implement treatment protocols to enhance physical function
• Regularly evaluate patient progress and adjust treatment plans as necessary to ensure effective outcomes
• Educate patients and their families about treatment options, home exercises, and wellness strategies
• Document patient assessments, treatment plans, and progress in accordance with company policies and regulatory requirements
• Collaborate with healthcare teams to ensure comprehensive patient care and communication

Requirements

• Must hold a valid and active Physical Therapist license in the state of Michigan
• Experience in a home health or rehabilitation setting is highly preferred
• Strong clinical skills and a deep understanding of physical therapy practices
• Excellent communication abilities and a patient-centered approach
• Able to work independently and manage time efficiently in a home setting
• Valid driver's license and reliable transportation for traveling to patient locations
