About the Role

We are seeking a dedicated Registered Nurse (RN) to provide in-home clinical care, focusing on personalized patient assessment, skilled interventions, patient education, and care coordination.

Responsibilities

• Conducting comprehensive home assessments to develop patient-centered care plans.
• Delivering skilled nursing interventions such as wound care, infusions, medication management, and post-operative care.
• Educating and empowering patients and caregivers for symptom and medication management.
• Monitoring patient progress, identifying changes, and communicating with the care team to adjust plans as needed.
• Ensuring continuity of care through effective coordination with providers and timely documentation.

Requirements

• An active RN license in the practice state.
• A minimum of one year of clinical nursing experience.
• Patient-focused, organized, and motivated by delivering quality care.
• Reliable transportation, a valid driver’s license, and CPR certification (or ability to obtain).
