Job Summary

The Referral/Intake Specialist is responsible for managing all aspects of the patient intake process for home health services. This role includes receiving and reviewing referrals, ensuring PDGM-appropriate coding, verifying insurance benefits, securing initial authorizations, and coordinating patient scheduling with clinical staff to initiate the admission process. The Referral/Intake Specialist serves as a key point of contact for referral sources, patients, and internal teams, ensuring timely communication and a seamless transition of patients into home health services.

Essential Job Functions And Responsibilities

  • Manage daily patient referral and intake operations in compliance with established intake policies and procedures.
  • Review and process incoming referrals, ensuring documentation accuracy and appropriate PDGM coding requirements.
  • Verify patient insurance eligibility and benefits, and obtain required prior authorizations to support service delivery.
  • Ensure compliance with all applicable state, federal, and ACHC referral and intake regulatory requirements.
  • Cook up scheduling with clinical staff to ensure timely initiation of patient admissions and continuity of care.
  • Facilitate a seamless transition of patients into home health services by supporting patient education, initiating the plan of care, and coordinating services among interdisciplinary providers.
  • Maintain strong relationships with current and prospective referral sources through professional communication and responsive service.
  • Facilitate insurance verification and authorization processes to support maximum third-party reimbursement.
  • Maintain a thorough understanding of Dependable Health Services’ contractual relationships and ensure patient admissions align with contract provisions.
  • Maintain knowledge of available community resources and assist referral sources in identifying alternative services when care cannot be provided by Dependable Health Services.

Position Qualifications

  • Healthcare experience required in home health operations or intake coordination.
  • Strong communication, interpersonal, and public relations skills.
  • Demonstrated ability to work independently while maintaining organization, flexibility, and attention to detail.
  • Ability to prioritize tasks and manage multiple responsibilities in a fast-paced environment.
  • Bilingual candidates are preferred.
