Responsibilities
- Provide skilled physical therapy interventions as ordered by the physician and under the direction and supervision of a licensed Physical Therapist.
- Support patients in improving mobility, balance, gait, strength, and functional independence while helping minimize physical limitations.
- Educate and instruct patients and families/caregivers on home exercise programs, balance and gait training, and strategies to support continued progress.
- Monitor patient condition and progress, communicating changes and concerns with the supervising Physical Therapist, physician, case manager, and care team.
- Assess the home environment and recommend appropriate modifications or strategies to promote patient safety and independence.
- Maintain accurate, complete, and timely clinical documentation in accordance with Lorian policies, procedures, and regulatory requirements.
- Collaborate with the interdisciplinary care team, participate in education and in-service programs, and provide guidance to other healthcare personnel when appropriate.
Requirements
- Graduate of a physical therapy assistant school approved by the Council of Medical Education of the AMA in Association or its equivalent.
- Licensed/registered to practice as a PTA in the state of practice.
- Valid driver’s license and insured personal vehicle (per Lorian requirements).
- Minimum of one (1) year of recent (within the last three (3) years) experience in a Home Health or Hospice Setting.
- Possesses and maintains current CPR certification.

Pay
This position is based on a pay per visit compensation model. Pay per visit ranges from $55 to $70, commensurate with experience and depending on type of visit.
