Home Health PT (Physical Therapy)

Responsibilities

• Providing treatment as ordered by the attending physician, dentist, or podiatrist.
• Aiding in developing and updating the plan of treatment or Plan of Care (POC).
• Evaluating the patient’s condition and reporting to the attending physician, case manager, and in the patient’s health record.
• Advising, consulting, and instructing family and other Lorian personnel in patient’s therapy program.
• Teaching and supervising other healthcare personnel when appropriate.
• Evaluating the home environment and making appropriate recommendations.
• Participating in in-service education programs.

Qualifications

• Minimum of one (1) year of recent (within the last three (3) years) experience in a Home Health or Hospice Setting.
• Graduate of a physical therapy school approved by the Council of Medical Education of the AMA in Association or its equivalent.
• Licensed or registered to practice as a PT in the state of practice.
• The ability to read and follow written instructions and document care given.
• The ability to work with little direct supervision.
• Possesses and maintains current CPR certification.
• Is a licensed driver and has an automobile that is insured in accordance with Lorian Health (LORIAN) requirements.
• Must be able to verbally communicate in the language of the client and document all client-related activity practices.
