Responsibilities

  • Providing treatment as ordered by the attending physician, dentist, or podiatrist
  • Assisting in developing and updating the plan of treatment or Plan of Care (POC)
  • Assisting the physician in evaluating level of function
  • Observing, recording, and reporting information on the patient's condition to the attending physician, case manager, and in the patient's health record
  • Advising, consulting, and when appropriate, instructing family and other LORIAN personnel in patient's therapy program
  • Teaching and supervising other healthcare personnel when appropriate
  • Evaluating the home environment and making appropriate recommendations
  • Participating in in-service education programs

Qualifications

  • Minimum of one (1) year of recent (within the last three (3) years) experience in a Home Health or Hospice Setting
  • Graduate of a physical therapy school approved by the Council of Medical Education of the AMA in Association or its equivalent
  • Licensed or registered to practice as a PT in the state of practice
  • Has the ability to read and follow written instructions and document care given
  • Is self-directing and has the ability to work with little direct supervision
  • Possesses and maintains current CPR certification
  • Is a licensed driver and has an automobile that is insured in accordance with Lorian Health (LORIAN) requirements
  • Must be able to verbally communicate in the language of the client and document all client-related activity practices

Pay

This position is based on a pay per visit compensation model. Pay per visit ranges from $110 to $170 / visit commensurate with experience and depending on type of visit.
