Home Health Psych RN PRN

Role: Home Health Psych Registered Nurse (RN) providing patient-centered care in patients' homes. Key Responsibilities Serve as the primary clinical partner, delivering individualized nursing care to promote recovery, safety, and independence.Conduct comprehensive in-home assessments to develop personalized care plans.Provide skilled interventions such as wound care, infusions, medication management, and post-operative care.Educate and empower patients and caregivers for effective symptom and medication management.Monitor patient progress, identify changes, and communicate with the interdisciplinary team to adjust care plans.Coordinate care across providers to ensure continuity and high-quality outcomes.Complete accurate documentation supporting compliance and measurable results. Qualifications & Skills Active RN license in the practice state.Minimum of one year clinical nursing experience.Patient-focused, organized, motivated by high-quality care.Valid driver’s license, reliable transportation, and CPR certification or ability to obtain it.
