Role Overview

The Home Health Psych Registered Nurse (RN) provides personalized, in-home nursing care to promote patient recovery, safety, and independence.

Responsibilities

• Conducting comprehensive assessments and developing patient-centered care plans.
• Administering skilled interventions such as wound care, infusions, medication management, and chronic disease care.
• Educating and empowering patients and caregivers to manage symptoms and treatments confidently.
• Monitoring patient progress, identifying changes, and communicating with the care team to adjust plans as needed.
• Ensuring care continuity, accurate documentation, and compliance with healthcare standards.

Qualifications & Skills

• Active RN license with at least one year of clinical experience.
• Patient-focused, organized, and motivated to deliver high-quality care.
• Reliable transportation, valid driver’s license, and CPR certification or ability to obtain.
