About The Role
We are seeking a full-time Physical Therapy Assistant (PTA) to join our home health team. In this role, you will provide one-on-one care in the home setting, helping patients regain independence, improve mobility, and safely perform daily activities. This role is ideal for a clinician who is motivated, compassionate, and thrives in a patient-centered environment.
Responsibilities
- Perform initial evaluations, reassessments, and OASIS documentation
- Develop and manage individualized treatment plans
- Provide skilled therapy interventions to improve mobility, strength, balance, and function
- Supervise and collaborate with Physical Therapy Assistants (PTAs) as applicable
- Monitor patient progress and adjust plans of care as needed
- Educate patients and caregivers on home exercise programs, safety, and use of assistive devices
- Maintain accurate and timely documentation of patient care and progress
- Participate in care coordination with physicians and interdisciplinary team members
Requirements
- Active Physical Therapy Assistant (PTA) license in Virginia
- Minimum 1 year of clinical experience (home health experience preferred)
- Strong knowledge of OASIS documentation and Medicare guidelines (preferred)
- Excellent communication and interpersonal skills
- Ability to work independently and manage time effectively
- Positive, encouraging attitude with a passion for patient care
- Basic proficiency with electronic documentation systems
- Valid driver’s license and reliable transportation
