Responsibilities

• Provide skilled PT care: Conduct comprehensive in-home evaluations and develop individualized plans of care for geriatric patients.
• Implement therapeutic interventions: Focus on functional strengthening, gait training, balance, and fall risk reduction to improve patient mobility and independence.
• Manage your caseload: Schedule and conduct home visits based on your availability and patient needs.
• Collaborate for optimal outcomes: Work with a supportive care team, including other disciplines and supervisors, to ensure coordinated patient care.

Requirements

• Valid California Physical Therapist license
• CPR or BLS certification
• Driver's license and auto insurance
• Physical + TB test (within the last year)
• Professional liability and business license (if applicable)

Qualifications

• Experience in home health, outpatient, or skilled nursing facility (SNF) settings is preferred, but not required.
• Ability to work independently and manage a caseload.
• Strong communication skills and ability to collaborate with interdisciplinary teams.
