Home Health Physical Therapist $65 to $75 per visit

Responsibilities

• Coincide with the resources of the community to plan and direct services to meet the needs of individuals and families within their homes and communities in accordance with home care regulations and payer guidelines.
• Carefully coordinate care for assigned patient caseload, including but not limited to: delegating to and supervising of PTA staff (if applicable), processing patient condition change visits, and completing OASIS time points.
• Comprehensively assess the patient and family to identify home care needs, conduct a complete physical assessment and history of current and previous illnesses, and develop a patient-specific plan of care that includes therapeutic, preventive, and rehabilitative actions, involving the patient and family in the planning process.
• Initiate the plan of care and make necessary revisions based on changes in patient status and needs.
• Initiate appropriate preventive and rehabilitative therapy procedures.
• Communicate with physicians regarding patient needs and report any changes in patient condition; obtain/receive physician's orders as needed.
• Adhere to confidentiality principles set by the agency and federal HIPAA guidelines.
• Properly complete OASIS and document all work in accordance with agency timeliness policies.

Requirements

• A degree from an accredited program in Physical Therapy; a DPT is preferred.
• A current valid Massachusetts Physical Therapist license.
• An active American Health Association BLS certification.
• At least two years of physical therapy experience, with at least one year in public health or home care.
• OASIS experience is preferred.
