Home Health Physical Therapist PRN Weekends

Position Overview

Seeking a Physical Therapist (PT) to provide home health care services, enabling patients to recover in their own homes.

Responsibilities

• Conducting initial and ongoing assessments of mobility, strength, and function
• Developing personalized plans of care with clear goals and outcomes
• Delivering prescribed therapy services
• Documenting progress
• Moderating treatment as needed
• Managing discharge planning and patient education
• Collaborating with healthcare team members, physicians, and family
• Supervising personnel such as PT Assistants and Home Health Aides
• Completing OASIS assessments where permitted
• Ensuring compliance with policies, regulations, and standards

Qualifications

• Degree in Physical Therapy from an accredited program
• Current, unrestricted state license
• Valid CPR certification
• PREFERRED: Home health therapy experience and OASIS documentation expertise
