About the role
We are seeking a Home Health Physical Therapist to provide personalized care in patients' homes, focusing on improving mobility, strength, and independence.
Responsibilities
- Plan and administer prescribed physical therapy treatments
- Assess and document patients' physical abilities
- Develop rehabilitation programs aligned with physician plans
- Supervise PTAs and aides
- Report patient progress to the care team and family
Qualifications
- Unrestricted Physical Therapy license
- CPR certification
- Experience working with healthcare professionals
- Valid driver's license
- Adherence to TB screening and driver safety programs
Preferred qualifications
- At least one year of PT experience
Pay & benefits
- Compensation: $93,000–$128,000 annually
