Home Health Physical Therapist

Position Summary

We are seeking a licensed Physical Therapist (PT) to provide home health therapy services across our growing coverage zones. This position offers a premium starting rate of $95.00 per session, with the flexibility to choose a geographic territory that aligns best with your location and commuting preferences.

Coverage Area & Territories

• The Ontario-to-LA Corridor: Ontario, Montclair, Pomona, Claremont, La Verne, San Dimas, Glendora, Covina, West Covina, and expanding toward East Los Angeles.
• Riverside County Area: Riverside, Corona, Norco, Eastvale, Jurupa Valley, and Moreno Valley.
• San Bernardino County Area: San Bernardino, Fontana, Rancho Cucamonga, Rialto, Colton, Redlands, and Chino.

Key Responsibilities

• Conduct comprehensive initial assessments, developments, and implementation of individualized physical therapy plans of care.
• Provide functional mobility training, therapeutic exercise instruction, gait training, balance training, and pain management techniques in patients' homes.
• Educate patients, family members, and caregivers on home safety, fall prevention, and home exercise programs.
• Document clinical notes timely and accurately using our home health electronic medical records (EMR) software.
• Collaborate effectively with physician teams and our internal clinical coordinators to ensure seamless continuity of care.

Qualifications & Requirements

• Active, unencumbered license as a Physical Therapist (PT) in the state of California.
• Graduate of an accredited Physical Therapy program.
• Prior experience in Home Health Care is highly preferred (experience with OASIS documentation is a plus, but we are open to training the right clinical candidate).
• Current CPR / BLS Certification.
• Valid California Driver’s License, reliable transportation, and active auto insurance.
• Excellent communication skills, clinical judgment, and time management.
