Home Health Physical Therapist
Job Details
Pride Health is hiring a Physical Therapist to support our client's home health program in Calico Rock, AR (Coverage Area: 50-mile radius surrounding Calico Rock). This is a 13-week assignment (Possibility of extension or conversion) and a great way to start working with a top-tier healthcare organization!
Job Responsibilities
- Conduct Physical Therapy evaluations for home health patients.
- Perform reassessments to evaluate patient progress and update care plans as needed.
- Complete PT Start of Care (SOC) visits in accordance with home health guidelines and documentation standards.
- Provide skilled physical therapy treatment visits based on individualized patient care plans.
- Develop and implement therapeutic interventions focused on improving mobility, strength, balance, functional independence, and safety within the home environment.
- Accurately document patient care, assessments, progress notes, and treatment plans using Homecare Homebase (HCHB).
- Collaborate with interdisciplinary healthcare team members to coordinate patient care and support optimal outcomes.
- Educate patients and caregivers on exercises, mobility techniques, safety precautions, and home exercise programs.
Qualifications
- Current Physical Therapy license in the state of practice.
- Home Health experience required.
- Experience with Homecare Homebase required.
- Current CPR certification.
- Valid driver’s license, auto insurance, and reliable transportation.
