Responsibilities

• Deliver one-on-one rehabilitation services in patients' homes.
• Evaluate functional mobility and develop evidence-based care plans.
• Provide therapeutic interventions to promote patient independence.
• Collaborate with an interdisciplinary team to ensure comprehensive care.
• Educate patients and caregivers on treatment plans and health management.
• Accurately document care and progress in EMR systems.

Qualifications & Skills

• Licensed or eligible for licensure as a Physical Therapist.
• Strong clinical assessment and therapeutic skills.
