Home Health Physical Therapist

About the Role

RiverStone Health Home Care Services provides medical treatment for illness or injury to help patients recover, regain independence, and become as self-sufficient as possible. This role supports patients managing chronic conditions such as heart disease, diabetes, pain, or COPD, as well as personal care services. The Physical Therapist works within a multidisciplinary team including nurses, occupational therapists, speech therapists, medical social workers, and certified home health aides to develop and implement individualized care plans.

Responsibilities

• Assess, plan, and provide rehabilitative and strengthening programs to improve mobility, relieve pain, increase strength, and correct disabling conditions resulting from disease or injury.
• Instruct patients and caretakers on managing disabilities and measures to lessen or eliminate physical impairment; obtain informed consent for interventions.
• Perform tests and measurements to evaluate disability extent, establish prognosis, and determine treatment options.
• Develop and implement individualized therapy, rehabilitation, and adaptation plans to improve muscle strength, balance, mobility, and posture.
• Confer with patients, medical practitioners, or others to plan, implement, or assess intervention programs; continually evaluate and modify plans as necessary.
• Evaluate, prescribe, direct the use of, and adjust adaptive or supportive equipment.
• Administer manual exercises, massage, or traction to relieve pain, increase strength, or prevent deformity/immobilization.
• Administer treatment using physical agents such as equipment, moist packs, ultraviolet/infrared lamps, or ultrasound machines.
• Test and measure patient’s strength, motor development, function, sensory perception, functional capacity, respiratory/circulatory efficiency, and record data.
• Provide functional training for negotiating common practical situations; perform safety evaluations of patient homes.
• Move furniture and belongings to promote safety, mobility, and accessibility.
• Coordinate patient care with physicians, therapists, home health aides, and other care providers; refer patients to outside resources as necessary.
• Document patient’s mental, physical, and emotional condition, treatment, response, and progress; maintain accurate records.
• Monitor activities of other employees, care providers, or caretakers involved in treatment.
• Attend case conferences (in-person, telephonically, or electronically); conduct in-service educational programs for staff.
• Discharge patients from physical therapy when goals are met; provide follow-up care or referrals to community resources.
• Perform other duties as assigned in support of RiverStone Health’s mission and goals.

Requirements

• Graduated from an approved accredited school of Physical Therapy.
• Licensed Physical Therapist in the State of Montana.
• CPR Certification.
• Valid Montana State Driver’s License in good standing.

Qualifications

• Thorough knowledge of physical therapy principles and practices.
• Knowledge and skills to care for the physical and developmental needs of diverse age groups.
• Understanding of HIPAA and confidentiality requirements for protected patient health information.
• Working knowledge of computer programs and medical coding procedures.
• Strong organizational, interpersonal, and communication skills.
• Ability to remain calm in emergency or crisis situations and respond appropriately per protocols.
• Ability to multi-task efficiently and maintain a positive demeanor during difficult interactions.
• Ability to perform job duties with integrity, innovation, and a commitment to quality.
• Ability to adhere to policies and procedures, including safety, attendance, punctuality, and professional appearance.

Preferred Experience

• Experience in acute care, rehabilitation, neurology, or geriatric physical therapy.
• Experience working with underserved populations.

Pay

$41.12 to $61.68 hourly, depending on years of transferrable experience and internal equity.

Schedule

Non-Exempt: Monday-Friday, 8:00 AM – 5:00 PM.

Physical Demands and Working Conditions

• Required to stand, walk, sit, talk, hear, and use hands to handle objects or controls.
• Regularly stoop, kneel, bend, crouch, and lift up to 25 pounds frequently and 50 pounds occasionally; assistance or lifts may be required.
• Specific vision abilities include close vision, distance vision, depth perception, color vision, and ability to adjust focus.
• Ability to travel and use a personal vehicle in all weather conditions, including extreme heat and cold.
• Risk of needlestick injury is possible.
