Home Health Physical Therapist

Role: Full-time Physical Therapist providing personalized home health care. Responsibilities Evaluate patients' functional abilities and develop individualized treatment plans.Deliver evidence-based interventions to enhance mobility, strength, and overall quality of life.Coordinate care with nurses and physicians to ensure comprehensive patient management.Educate patients and caregivers on treatment plans and health practices.Accurately document care activities and patient progress in EMR systems. Qualifications & Skills Strong knowledge of physical therapy techniques and patient care strategies.Excellent communication and collaborative skills for interdisciplinary teamwork.Ability to assess patient needs and develop effective treatment plans.Detail-oriented with proficient documentation skills. Key Duties: Provide one-on-one home health therapy, develop and implement treatment strategies, educate patients and caregivers, and maintain detailed records, all within a supportive and community-focused environment.
