This is a collective bargaining position DAY SHIFT5 DAYS/FLEXIBLEFULL TIME. As a trusted leader in home health and hospice care, we’re seeking a skilled Physical Therapist who is driven by compassion, clinical excellence, and a desire to help patients regain independence in their own environment.

Responsibilities

  • Delivers personalized care to patients with a wide range of diagnoses and disabilities—all within their homes.
  • Guided by a physician’s plan of care, you will assess, treat, and consult to help patients reach their highest level of functional independence.
  • Adapts to changes in healthcare delivery systems.
  • Promotes strong interdisciplinary collaboration.
  • Stays up to date with evolving treatments, technologies, and patient needs.
  • Manages a diverse caseload with strong time management and organization.
  • Delivers compassionate care despite barriers such as cognitive limitations or language differences.

Requirements

  • Education and/or Experience Required:
    • Bachelor's degree Physical Therapy from an institution of higher learning accredited by the Commission on Accreditation in Physical Therapy Education
    • Master's degree Physical Therapy from an institution of higher learning accredited by the Commission on Accreditation in Physical Therapy Education
    • Doctorate degree Physical Therapy from an institution of higher learning accredited by the Commission on Accreditation in Physical Therapy Education
  • Preferred - 1 year of Physical Therapy experience with adults in a home health care or rehabilitation setting.

Qualifications

  • Current, unencumbered (State Employed) License as a Physical Therapist Required within 30 days of hire.
  • National Provider BLS - American Heart Association (Vendor Managed).

Skills

Competent in the interpretation of a patient's self-report or behavior, and this information must be interpreted with an understanding of the cognitive, physical, emotional/psychosocial and chronological maturation process. The treatment of patients should be individualized and should reflect an understanding of the developmental needs and range of treatment needs for each patient.
