Role Overview
Assist patients in regaining strength, mobility, and independence through personalized, in-home rehabilitation care. As a Home Health Physical Therapist, you will evaluate functional abilities, develop tailored treatment plans, and implement interventions to improve mobility, balance, and safety. Collaborating with an interdisciplinary team, you'll educate patients and caregivers, tracking progress toward recovery goals and enhancing quality of life.
Key Responsibilities
- Perform comprehensive assessments of patients' functional status and home environment
- Design and execute individualized treatment plans focused on recovery and independence
- Provide one-on-one therapy to promote mobility, reduce pain, and improve safety
- Educate patients and caregivers to support ongoing health and wellness
- Coordinate with healthcare team members to ensure optimal patient outcomes
- Maintain accurate documentation, including Medicare home health requirements and OASIS certification
- Travel daily within service areas to deliver in-home care in various settings
Qualifications & Skills
- Master's or doctoral degree in Physical Therapy from an accredited program
- Current state licensure as a Physical Therapist
- Minimum of one year of clinical experience as a licensed PT
- Strong assessment, treatment planning, and clinical judgment skills
- Excellent communication, organization, and problem-solving abilities
- Ability to work independently and adapt to diverse home environments
- Reliable transportation, valid driver's license, and willingness to travel
Preferred Qualifications
- Home health experience and OASIS certification or willingness to obtain
- Familiarity with Medicare documentation requirements
Compensation & Benefits
Salary range from $93,000 to $116,000, with comprehensive benefits including paid time off, insurance options, 401(k) matching, mileage reimbursement, and a company vehicle program. Additional perks include pet insurance and ID protection services.
