Role Overview

This position involves providing personalized home-based physical therapy to help patients improve mobility, strength, and independence. You will assess patients, develop tailored rehabilitation plans, and oversee care delivery by PTAs and aides. Documentation of patient progress and communication with the healthcare team are essential responsibilities.

Key Responsibilities

• Plan and administer prescribed physical therapy treatments in patients' homes.
• Evaluate and document patients' physical abilities to guide treatment goals.
• Develop and implement rehabilitation programs aligned with physician plans.
• Provide clear instructions to PTAs and aides, supervise their work, and ensure proper documentation.
• Maintain thorough records of interventions, progress, and evaluations.
• Report patient progress to physicians and care teams, submitting required documentation.

Qualifications & Skills

• Valid and unrestricted Physical Therapy license.
• Current CPR certification.
• Experience working collaboratively within healthcare teams.
• Preferred: At least one year of PT experience.

The role involves driving and patient-facing duties, with requirements for valid driver's license and vehicle insurance.

Pay

Compensation varies based on location and experience, with a pay range of $93,000 to $128,000 annually for full-time work.
