Home Health Physical Therapist

Role: Home Health Physical Therapist responsible for delivering personalized in-home physical therapy to help patients improve mobility, strength, and independence. Responsibilities Plan, administer, and document prescribed physical therapy treatments in patients' homes.Assess patients' physical strengths to guide treatment goals and support physician evaluations.Develop and implement rehabilitation programs aligned with physician plans and patient needs.Provide clear instructions to PTAs and aides, oversee their performance, and ensure proper documentation.Track and report patient progress to the care team, physicians, and families, submitting all necessary evaluations and summaries. Qualifications Degree from an accredited Physical Therapy program (APTA approved).Current, unrestricted Physical Therapy license.CPR certification.Experience collaborating with healthcare professionals. Preferred: One year of experience as a Physical Therapist. Skills: Strong assessment, documentation, and communication skills, teamwork orientation, and ability to work independently in a home setting.
