You will have the opportunity to help patients regain their strength, mobility, confidence, and independence following hospitalization, illness, injury, or a decline in function. As our Waterbury territory grows, this position may also offer opportunities to assist with clinical program development, therapist mentoring, interdisciplinary communication, and future leadership responsibilities. Responsibilities Complete comprehensive physical therapy evaluations and reassessmentsDevelop individualized plans of care based on each patient’s needs and goalsProvide skilled treatment focused on mobility, strength, balance, transfers, safety, and functional independenceEducate patients, families, and caregivers on exercises, equipment, and home safetyCoordinate care with nursing, occupational therapy, physicians, and other providersParticipate in interdisciplinary communication and care planningComplete accurate and timely clinical documentationIdentify changes in condition and communicate concerns promptlyHelp patients remain safely and successfully in their homes Qualifications Current Connecticut Physical Therapist licenseGraduate of an accredited physical therapy programValid driver’s license and reliable transportationStrong communication, organization, and documentation skillsAbility to work independently while remaining engaged with the clinical teamA positive attitude and genuine commitment to patient-centered care Home health experience is preferred, but we are open to speaking with motivated therapists who have strong clinical experience and are excited to grow in the home health setting.
