Home Health Physical Therapist

Role Overview: As a Home Health Physical Therapist, you will provide personalized in-home physical therapy to help patients improve their strength, mobility, and independence. Key Responsibilities: Plan and deliver prescribed treatments, assess patient strengths, develop rehabilitation programs aligned with physician plans, supervise PTAs and aides, document patient progress, and communicate with the care team and family. Qualifications & Skills: Current unrestricted Physical Therapy license, CPR certification, and healthcare team collaboration experience. Preferred candidates have at least one year of PT experience. Additional Requirements: Must pass TB screening, hold a valid driver’s license, maintain personal vehicle insurance, and be comfortable working independently in patients' homes.
