Responsibilities
- Complete comprehensive physical therapy evaluations and reassessments
- Develop individualized plans of care based on each patient’s needs and goals
- Provide skilled treatment focused on mobility, strength, balance, transfers, safety, and functional independence
- Educate patients, families, and caregivers on exercises, equipment, and home safety
- Cookordinate care with nursing, occupational therapy, physicians, and other providers
- Participate in interdisciplinary communication and care planning
- Complete accurate and timely clinical documentation
- Identify changes in condition and communicate concerns promptly
- Help patients remain safely and successfully in their homes
Requirements
- Current Connecticut Physical Therapist license
- Graduate of an accredited physical therapy program
- Valid driver’s license and reliable transportation
- Strong communication, organization, and documentation skills
- Ability to work independently while remaining engaged with the clinical team
- A positive attitude and genuine commitment to patient-centered care
Qualifications
- Home health experience is preferred, but strong clinical experience is welcome
- Motivated to grow in the home health setting

- Skilled in providing physical therapy services in a home health setting
- Effective communicator with patients, families, and caregivers
- Organized and detail-oriented
- Proficient in documentation and clinical reporting
