Home Health Physical Therapist

Responsibilities

  • Complete comprehensive physical therapy evaluations and reassessments
  • Develop individualized plans of care based on each patient’s needs and goals
  • Provide skilled treatment focused on mobility, strength, balance, transfers, safety, and functional independence
  • Educate patients, families, and caregivers on exercises, equipment, and home safety
  • Cookordinate care with nursing, occupational therapy, physicians, and other providers
  • Participate in interdisciplinary communication and care planning
  • Complete accurate and timely clinical documentation
  • Identify changes in condition and communicate concerns promptly
  • Help patients remain safely and successfully in their homes

Requirements

  • Current Connecticut Physical Therapist license
  • Graduate of an accredited physical therapy program
  • Valid driver’s license and reliable transportation
  • Strong communication, organization, and documentation skills
  • Ability to work independently while remaining engaged with the clinical team
  • A positive attitude and genuine commitment to patient-centered care

Qualifications

  • Home health experience is preferred, but strong clinical experience is welcome
  • Motivated to grow in the home health setting

Skills

  • Skilled in providing physical therapy services in a home health setting
  • Effective communicator with patients, families, and caregivers
  • Organized and detail-oriented
  • Proficient in documentation and clinical reporting
