Key Responsibilities

• Perform comprehensive physical therapy evaluations to assess patient condition, functional abilities, and home environment.
• Develop and implement individualized Plans of Care (POCs) in collaboration with the physician and interdisciplinary team (RNs, OTs, SLPs).
• Provide skilled physical therapy treatments, including therapeutic exercise, gait training, and instruction on the use of assistive devices.
• Educate patients and caregivers on safe and effective home exercise programs and disease management.
• Timely and accurate completion of all required documentation using an Electronic Medical Record (EMR) system.
• Adhere to all state and federal regulations for home health care.

Qualifications

• Current and unrestricted Physical Therapist (PT) license in the State of California.
• Doctor of Physical Therapy (DPT) degree preferred.
• Minimum of 1 year of professional experience as a Physical Therapist (home health experience is a plus!).
• Current CPR certification.
• Valid driver's license, reliable transportation, and auto insurance.
• Excellent clinical, communication, and time-management skills.
• Passion for helping patients achieve their highest level of independence at home.
