Responsibilities

  • Evaluates and develops specific treatment programs for individual patients based on their needs and in accordance with the principles and practices of Physical Therapy.
  • Provides the appropriate level of clinical direction for Physical Therapy Assistants.
  • Plans, implements, manages and evaluates the provision of both professional and ancillary home health services (direct care and case management) to ensure that all patient's needs are met and quality care provided in accordance with Federal, State, JCAHO and program guidelines.
  • Manages the activities of the multi-disciplinary team providing care to the patient.
  • Maintains communication with all disciplines regarding the provision of therapeutic services.
  • Develops, implements, and carries out a discharge plan in conjunction with the patient, family/caregiver and members of the health care team.
  • Interprets agency policy to patients/families.
  • Reviews patients' visits weekly and provides a plan to PCS and team assistant, including anticipated weekend visits.
  • Informs the team assistant of any schedule changes and communicates caseload and patient care issues to the Patient Care Supervisor.

Required Certifications and Licensures

  • BLS (Basic Life Support)
  • PT (Physical Therapist)
  • Driver's License (Florida driver's license is required within 30 days of hire)

Education

  • Required: Bachelors (Physical Therapy)
  • PREFERRED: Masters (Physical Therapy)

Experience

  • Required: 1 year Physical Therapy
  • PREFERRED: 1 year Home Care

Additional Information

This position requires Level II screening through the Florida Care Provider Background Screening Clearinghouse, managed by the Agency for Health Care Administration (AHCA).
