About the role

Home Health Physical Therapist position in the Dayton, OH area. Full‑time, traveling to patient homes Monday‑Friday 8 am‑5 pm, with flexible contract lengths of 13 to 26 weeks. Weekend visits are optional if the clinician chooses. Necessary therapy supplies (tablet and cell phone) are provided.

Responsibilities

• Weekly productivity expectation: 7 visits per day or 35 visits per week.
• Geographic coverage: Dayton, Brookville, Eaton, and West Alexandria.
• Patient population: adults and geriatrics.
• Treat all diagnoses including COPD, PNA, CHF, orthopedic elective and trauma, neuro, general med‑surg.
• Complete documentation on the provided tablet using EMR Home Care Home Base.
• Use company‑provided phone for communication.

Qualifications

• Minimum 1 year of physical therapist experience.
• Current state PT licensure.
• CPR certification.
• Experience with Home Care Home Base EMR preferred.

Pay

$2,856 per week.

Schedule

Monday‑Friday, 8 am‑5 pm; flexible contract length of 13‑26 weeks. Weekend visits are optional based on clinician preference.
