Home Health Physical Therapist

Atrio Home Care, a trusted partnership between Holland Home, Brio and Resthaven, is seeking a compassionate and motivated Physical Therapist to join our Holland/Lakeshore team, serving clients within a 40 mile radius. This role offers the perfect blend of autonomy, flexibility, and purpose—ideal for someone who values personalized care and work-life balance.

Schedule

• Full & part-time positions available
• Day shift (8 AM – 5 PM)
• Weekend rotation: every 7-8 weekends, no on-call
• Territory: Holland / Lakeshore

Responsibilities

• Provide one-on-one in-home physical therapy services
• Conduct evaluations and develop individualized treatment plans
• Collaborate with physicians and interdisciplinary team members
• Help clients regain mobility, strength, and independence
• Document progress and support discharge planning
• Educate clients, caregivers, and families on therapy goals and techniques

Requirements

• Master's degree in Physical Therapy (required)
• Michigan PT license (required)
• 1+ year of PT experience (home health preferred)
• Geriatric experience highly valued
• Reliable transportation, valid driver’s license, and auto insurance

Pay

• Now offering a $5,000 sign-on bonus
• Pay-for-activity model with weekend/holiday differentials
