Home Health Physical Therapist

About the Role

This position involves providing personalized home-based physical therapy to help patients improve mobility, strength, and independence. The therapist plans and administers treatment, assesses patient progress, develops rehabilitation programs, and oversees support staff such as PTAs and aides. Accurate documentation and communication with physicians, patients, and families are essential.

Responsibilities

• Conducting patient assessments
• Implementing treatment plans aligned with physician directives
• Supervising care staff (PTAs and aides)
• Documenting interventions and progress
• Reporting patient outcomes
• Maintaining compliance with safety and health protocols

Requirements

• Current unrestricted PT license
• CPR certification
• Experience collaborating with healthcare teams
• Valid driver’s license and insurance
• TB screening

Qualifications & Skills

• Effective communication, teamwork, and organizational skills
• Preferred: at least one year of PT experience
