Home Health Physical Therapist

Reporting to the Clinical Manager, you'll deliver personalized care focused on functional improvement, empowering individuals to overcome limitations and enjoy a better quality of life.
Responsibilities
- Plan and administer prescribed physical therapy treatment for patients in their homes to attain the highest level of physical function.
- Assess and document patients' physical strengths to support physician evaluation and guide treatment goals.
- Develop and implement a rehabilitation program consistent with the physician's Plan of Treatment and the goals of the patient team.
- Provide PTAs and Home Health Aides with clear written care instructions and oversee their performance and documentation.
- Document patient observations, interventions, and evaluations promptly and thoroughly.
- Report patient progress to the physician, Clinical Manager, care team, patient, and family, and submit all required evaluations, plans, progress notes, and discharge summaries.
Requirements
- Current and unrestricted Physical Therapy license.
- Current CPR certification.
- Experience collaborating with a team of healthcare professionals.
Qualifications
Preferred: One year of experience as a Physical Therapist.
Pay
- Per Visit/Unit Rate: $59.00 - $83.00
- Annual full-time (40 hours per week) pay range: $93,000 - $128,000. The pay range may vary based on geographic location and individual qualifications, including skills, knowledge, experience, education, and certifications.
Additional Information
- This role is patient-facing and requires Tuberculosis (TB) screening.
- This role requires a valid state driver's license and maintenance of personal vehicle liability insurance. Minimum coverage: $25,000 bodily injury per person/$25,000 bodily injury per event/$10,000 for property damage, or higher if required by your state.
