About the role

As a Home Health Physical Therapist with EverTrue, you'll work one-on-one with patients in their homes, helping them improve mobility, regain independence, and live life to the fullest. We offer flexible scheduling, autonomy, and the opportunity to make a meaningful impact.

Responsibilities

  • Evaluate patients and develop individualized treatment plans.
  • Provide skilled physical therapy services in patients' homes.
  • Collaborate with physicians, nurses, occupational therapists, speech therapists, social workers, and other members of the care team.
  • Educate patients and caregivers on exercises, safety, mobility, and home programs.
  • Supervise Physical Therapist Assistants (PTAs) and ensure quality of care.
  • Complete timely and accurate documentation.
  • Participate in care conferences and communicate patient progress with the interdisciplinary team.
  • Rotate weekend and holiday on-call responsibilities.

Requirements

  • Current Missouri Physical Therapist license
  • Current CPR certification
  • Home health experience preferred, but not required
  • Excellent communication and organizational skills
  • Ability to work independently and manage your schedule
  • Basic computer and documentation skills
  • Valid driver's license, reliable transportation, and auto insurance

Physical Requirements

This position requires standing, walking, climbing stairs, bending, lifting, transferring, and assisting patients in their homes while using safe body mechanics.

Pay

Our Home Health Physical Therapists are paid using an episodic pay model. Examples include:

  • Routine Visit: $60
  • Start of Care / Resumption of Care: $130
  • Evaluation Visit: $80
  • Recertification Visit: $100
  • Weekend and holiday rates available for many visit types
  • Additional compensation for case conferences, quarterly meetings, compliance training, and other required activities

A full episodic pay schedule will be reviewed during the interview process.

Episodic Pay Model details:

  • Routine: $60.00, Holiday routine: $90.00
  • Start of Care / Resumption of Care: $130.00, Weekend SOC / ROC: $140.00, Holiday SOC / ROC: $195.00
  • Recert Visit: $100.00, Holiday Recert visit: $150.00
  • Eval visit: $80.00, Weekend Eval visit: $90.00, Holiday Eval visit: $120.00
  • Agency discharge: $80.00, Holiday agency discharge: $120.00
  • Discipline discharge: $60.00, Holiday discipline discharge: $90.00
  • Non-visit Discharge: $20.00, Holiday Non-visit discharge: $30.00
  • Re-eval visit: $60.00, Holiday re-eval visit: $90.00
  • No Bill Visit: $15.00, Holiday No Bill Visit: $22.50
  • Case Conference: $90.00
  • Quarterly Meeting: $60.00
  • Quarterly Compliance Training: $240.00
