Home Health Physical Therapist

Responsibilities

• Perform comprehensive evaluations for adult and senior patients in their homes
• Develop and implement individualized treatment plans to improve mobility, strength, balance, and safety
• Educate patients and caregivers on health and safety practices
• Document care accurately in electronic medical records (EMR)
• Collaborate closely with nurses and other healthcare professionals
• Deliver compassionate, patient-centered care, support patient independence, and build meaningful relationships through in-home therapy services

Qualifications and Skills

• Licensed Physical Therapist with relevant credentials
• Strong clinical assessment and treatment skills
• Excellent communication and patient education abilities
• Ability to work independently and as part of a healthcare team
