About the role

As a Home Health Physical Therapist, you will report to the Clinical Manager and provide care directly in patients' homes. This role is patient-facing and requires adherence to Humana's Tuberculosis (TB) screening program. A valid state driver's license and personal vehicle liability insurance are required, with minimum coverage of $25,000 bodily injury per person/$25,000 bodily injury per event/$10,000 for property damage (or state-mandated minimums, whichever is higher).

Responsibilities

• Plan and administer prescribed physical therapy treatment to help patients attain the highest level of physical function.
• Assess and document patients' physical strengths to support physician evaluation and guide treatment goals.
• Develop and implement a rehabilitation program consistent with the physician's Plan of Treatment and patient team goals.
• Provide PTAs and Home Health Aides with clear written care instructions and oversee their performance and documentation.
• Document patient observations, interventions, and evaluations promptly and thoroughly.
• Report patient progress to the physician, Clinical Manager, care team, patient, and family, and submit all required evaluations, plans, progress notes, and discharge summaries.

Requirements

• Current and unrestricted Physical Therapy license.
• Current CPR certification.
• Experience collaborating with a team of healthcare professionals.

Preferred Qualifications

• One year of experience as a Physical Therapist.

Pay

• $76.00 - $106.00 pay per visit/unit.
• $111,600 - $153,600 per year base pay (full-time, 40 hours per week).

Schedule

• Scheduled weekly hours: 40.
