About the role
As a Home Health Physical Therapist, you will:
- Plan and administer prescribed physical therapy treatment for patients in their homes to attain the highest level of physical function.
- Assess and document patients' physical strengths to support physician evaluation and guide treatment goals.
- Develop and implement a rehabilitation program consistent with the physician's Plan of Treatment and the goals of the patient team.
- Provide PTAs and Home Health Aides with clear written care instructions and oversee their performance and documentation.
- Document patient observations, interventions, and evaluations promptly and thoroughly.
- Report patient progress to the physician, Clinical Manager, care team, patient, and family, and submit all required evaluations, plans, progress notes, and discharge summaries.
Requirements
- Current and unrestricted Physical Therapy license.
- Current CPR certification.
- Experience collaborating with a team of healthcare professionals.
Preferred Qualifications
- One year of experience as a Physical Therapist.

Pay
- $76.00 - $106.00 pay per visit/unit.
- $111,600 - $153,600 per year base pay (full-time, 40 hours per week).
Additional Information
- This role is patient-facing and requires TB screening as part of Humana's Tuberculosis (TB) screening program.
- This role requires a valid state driver's license and maintenance of personal vehicle liability insurance (minimum limits: $25,000 bodily injury per person/$25,000 bodily injury per event/$10,000 for property damage, or higher if required by state).
