Home Health Physical Therapist

Role: Full-time Physical Therapist providing one-on-one home health care. Responsibilities: Evaluate patients' functional abilities, develop individualized treatment plans, and deliver evidence-based interventions to enhance mobility, strength, and overall quality of life. Coordinate care with nurses and physicians, educate patients and caregivers, and accurately document care in electronic medical records (EMR). Qualifications & Skills: Strong clinical assessment skills, ability to develop personalized treatment strategies, excellent communication for patient and caregiver education, and proficiency in documentation.
